According to Charaka, to successfully cure any Illness, the Four Pillars (Pada Chatushtayam) are required to restore the health of an individual (Pravruthir dhatu samyaardha chikitsetyabhidhiyathe).
They are: • Bhishak (Doctor), • Dravyam (Remedy or Medicine), • Upastha (Paricharika or Attendant) and • Rogi (Patient).
The Doctor tops the list because an incompetent doctor will stifle all of the best efforts of the remaining three, and even progress the illness further. A Doctor must be Skilled, Experienced and Knowledgeable, as well as pure in intention. He is very important in Pada Chatushtayam because he is a Pranabhisara (one who gets back health in a dying spirit). He also should have the qualities of Vidya (Shashtra Gyanam), Vitarka (Discernment), Vignanam (Theoretical Knowledge), Smrithi (Memory), Tatparatha (Dhyanam), Kriya (Good deeds). The Medicine will be appropriate to the Illness, the Patient and the Environment. It will utilize multiple avenues (Diet, Life Style, Medicine, Panchkarma etc), be readily available and excellent quality.
The Attendent will be Pure, Kind and Compassionate, as well as having a Strong understanding of Ayurveda and the Skill to put it into practice effectively.
The Patient, lastly, but certainly not the least, who must be Courageous and Faithful. An ideal patient is able to describe their symptoms accurately, and will follow the doctor’s instructions precisely.
With these Pada Chatushtayam the Health or Dhatu Samyavastha can be achieved. The stronger the Four Pillars, the earliest is the recovery from the disease.
